All Claims MUST be initiated by the insured. Remember to sign all forms and send back to the claims department.
To file a claim, simply email contact@volunteercard.com or call 952.886.7663. A trained staff member will be more than willing to walk you through the process.
Please have ready the following information:
- Volunteer Card number (begins with “V” and is found on the front of your card)
- Date of birth
- Travel dates and destination
- Date incident occurred / you were treated
- Brief description of incident / what you were seen for
- Amount claiming (in dollars)
You will receive an email within 24-48 business hours with a claim number and further instructions, including what documentation is needed to complete the claim.
